When you file bankruptcy do you have to have a copy of the debt or can you use the acct, number only?

I can’t find a bill that I need to put on my bankruptcy, but I have it listed on my credit report, and it’s been turned over to a collection agency (which I have a copy of) So since I know the comany’s address and the account number will that be acceptable?
